Journal of neonatal surgery
We present a case of heteropagus twins attached to the epigastric region. The neonate also had ruptured giant omphalocoele with most of gut and liver lying outside the abdominal cavity. Patient had uneventful surgery for separation of twins and repair of ruptured omphalocoele.

We present a case of conjoined omphalopagus twins in a 35-year-old woman who came for antenatal booking at 9 weeks of gestation to a health centre in Kota Bharu, Kelantan. The diagnosis was confirmed via ultrasonography by an obstetrician in a tertiary Hospital at 12 weeks of gestation. The pregnancy was allowed to progress as it carried the best chance of survival. However, the parents were in dilemma with the decision. Primary care practitioners play an important role to prepare patients mentally and physically to face the outcome of the pregnancy. Emergency Separation of Extreme VLBW Omphalopagus Twins: Case ReportThe classification of conjoint twins is based according to the site of attachment. The challenges in management of such anomalies span the entire continuum of care from delivery to resuscitation to separation and finally discharge. Scheduled separation is ideal, occasionally the caring team is faced with no option but to perform an emergent separation. Omphalopagus is a type of conjoined twinning characterized by union of the peritoneal cavities through an infraumbilical abdominal wall defect. In this report we describe our experience with a successful emergency separation of extremely preterm omphalopagus twins.This is the first case of conjoint twins in Kuwait, we highlight the challenges faced, stressing the importance of adhering to antenatal care as well as management by a multidisciplinary team.

Conjoined Twins: A Case Report of Thoraco-Omphalopagus2011 •
Conjoined twins are extremely rare congenital malformations. Any associated genetic, environmental or demographic factors have not been identified. The phenomenon occurs between the 13th and 15th day after fertilization due to failure in splitting of embryonic axis. Twins are classified according to the major site of union, with the suffix pagus meaning fixed or fastened. Omphalopagus twins are the second most common variety of conjoined twins and usually are joined at the umbilicus. A 31-year-old pregnant female admitted for routine gestational control. Obstetric ultrasound (US) revealed two fetuses which were adherent from abdominal region. Their livers were fused however other organs were separated. Hepatic veins and portal veins were separated in both fetuses on color Doppler evaluation. Fetal magnetic resonance imaging (MRI) confirmed the diagnosis as omphalopagus conjoined twins. Background: Conjoined twins are a rare type of monoamniotic twins, estimated to occur in 1.5 per 100,000 births worldwide [1]. The incidence appears remarkably similar throughout the world, although the report is higher from Atlanta 1 per 20,000 births [2]. When twins are conjoined, the fusion occurs between the same body parts. Conjoined twins are classified as cephalopagus, thoracopagus, omphalopagus, ischiopagus, parapagus, craniopagus, rachipagus and pygopagus based on the site of fusion. Case Report: Here we report a case of a 27-year-old Gravida 4 para 2 Abortion 1 mother who presented with labor and rupture of membrane. She had two previous cesarean deliveries. She did not have ultrasound scanning throughout her pregnancy. She was in active labor and an emergency Cesarean section was done. Gluteal flap for omphalocele repair in a case of epigastric heteropagus: A novel approach for surgical management2013 •
ABSTRACTEpigastric heteropagus is an extremely rare congenital anomaly, in which there is asymmetrical conjoined twinning, with the parasitic twin attached to the epigastrium. A 2-day-old male, with epigastric heteropagus and omphalocele, was operated in our institution. After excision of the parasitic twin, omphalocele was covered with a gluteal skin flap available from the parasitic twin. Post-operative course was uneventful, except for infection along the edges of the skin flaps, which was managed conservatively. Only 44 cases of epigastric heteropagus twins have been reported previously in world literature. We present a novel surgical approach for the repair of the omphalocele in a case of epigastric heteropagus twins, probably the 45 th case to be reported in the world literature.
